Health Insurance

 

STUDENT

Students from outside the EU are insured in Poland by concluding a voluntary health insurance contract with the National Health Fund (Narodowy Fundusz Zdrowia) and pay the health insurance premium themselves.
Students from outside the EU who have a Pole’s Card or a certificate of Polish origin, are going to be registered for health insurance by their university (provided that they do not have any other entitlement to compulsory health insurance).

You have just signed an agreement with the NFZ (National Health Fund) – what next:

  1. Go to ZUS (Social Insurance Institution) with the agreement. You have 7 days to do this !!!
    • Show the agreement to the ZUS employee
    • Submit the completed ZUS ZZA
    • Obtain the account numer to which you will pay your health insurance contributions.

  1. Pay the monthly health insurance contribution:

The monthly premium is paid „in arrears,” e.g., for OCTOBER until 20 NOVEMBER etc.

The amount of the monthly contribution is PLN 55.80
(basis for calculation – 620.00 PLN)

  1. Submit a monthly settlement declaration to ZUS – the ZUS DRA form – without this, we will not see your contribution,

and if you visa a doctor, you may be „flagged red”!!!

Want to terminate the agreement?

  1. Submit an application to terminate the voluntary agreement and a document confirming the completion of your studies to the NFZ. You can do this:

  1. Go to ZUS to deregister from the voluntary agreement. Submit the completed ZUS ZWUA and ZUS ZWPA

 

You have 7 days to do this !!!

IMPORTANT

If the termination application specifies the date 30 June 2024, then on the ZUS ZWUA form, indicate the date 01 July 2024.
